The four main pages of the collision report form are composed as one file. Supplemental pages are each in a separate file.
- The pre-printed forms and padded forms are to be completed by handwriting only. These forms may be obtained from DPS Supply Division as indicated below.
- The computer-fillable form must be downloaded to your computer to perform properly. The form, including a diagram, is to be filled out, printed, and mailed to DPS as usual. You will need the latest version of Adobe Acrobat Reader® which is available at no charge from Adobe's website.
- You will be able to use the "Save package as" command, under the "File" menu to save the first four pages as a group if you are using Adobe Reader 8 . If you are using Adobe Reader 7, each page will need to be saved individually.
- In either case, mail the completed form to DPS. Remember this form is specially designed to be scanned and digitally read by the Records Management Division at DPS and is the only officially authorized form. Self-generated forms are not acceptable. The better quality the form, the better quality scanning can be achieved.
