You must have a learner permit for a minimum of 6 months from the issue date. You have to go to a Driver License Exam station and perform your skills (drive) test with a Driver License Examiner.

You will need to bring the following:

1. Your Learner Permit
   Click here for optional Primary ID if you lost your permit.


2. Secondary ID
   Applicant must bring Secondary ID (Usually a Social Security Card)


3. Insurance Security Verification Form
   The Insurance Security Verification Form that has the VIN number matching the vehicle you will be driving.


4. Social Security Number


5. Driver Education Completion Certificate


6. Parent or Legal Guardian
   If the parent or legal guardian cannot be present they can download this form, complete it in the presence of a notary public and have the applicant bring it to the examiner.
   
   Note: The Vehicle must have a current tag, valid insurance and pass a vehicle inspection.
